To Fax in Powder Orders
A short-cut when ordering a formula is to specify
the formula's page in Bensky (Formulas & Strategies),
with additions or subtractions, if you wish.
Otherwise, write out the entire original formula and dosages,
and the patient's daily dosage and duration, and we'll do
all the math for you.
Granular dosing:
- High dosage = 5-10 scoops, 2 times per day
- Medium dose= 4 scoops, 2 times per day
- Low dosage = 3 scoops, 2 times per day
Re-orders: All formulas are kept on file. Simple call
or fax with patient's name and label information.
Liquids
How to order Liquid extracts:
- Fill out and fax the liquid
form to (877) 283-0678, or
- Call 866.206.9069 x5284 and include all the info requested
on the fax form

What dosage should I use?
For best compliance, no more than TWICE daily dosing is
recommended, unless it's an acute condition.
For Standard extracts:
- High dosage = 6 droppers, 2 time per day
- Medium dose= 4-5 droppers, 2 times per day
- Low dosage = 3 droppers, 2 times per day
For Super concentrates:
- Higher dose = 3-4 droppers, 2 times per day
- Lower dose = 1-2 droppers, 2 times per day
- Fill in, your mark-up, shipping fee, sales tax.

Refills
A Refill is a formula without changes to individual ingredients,
individual herb dosages, or liquid formula proportions.
We store every formula ordered by Patient Name and Formula
Number or Formula Name, so refills are a snap.

If modifications to the formula are required, we request
you use the on-line form or one of the fax forms.
